Please note that the Vendor Registration and Vendor Change forms on this site have been changed as of November 21, 2007.
The Vendor Registration form has been replaced by the State of Nevada Payee Registration - Substitute Form IRS W-9. The substitute IRS Form W-9 is for the use of United States entities only. Non-US entities must submit an IRS Form W-8. The Vendor Change form has been replaced by the Payee Change Form. This form is to provide updated information and/or additional remittance addresses for previously established payees of the State of Nevada. If you have questions, please contact Anna Verbanac.
